Not sure about password thing but you can try Kasper Sky antivirus because it will not let user open flash drive until user do not scan it. or intentionally don't want to scan it.

There are several ways to do so. They can assign bandwidth on D-Slam port on which you are connected or else they can do it via their billing system (billing will add a queue automatically against IP when user get authenticated.

If you are in office network then better solution is to use domain controller to restrict the user for being changing ip and mac or else use a manageable switch if you can go with active directory.
